What Exactly is an Eye Chart?

If there’s one aspect of optometry that everyone recognizes, it’s the traditional eye chart, with its rows of big letters on top, which gradually become smaller the farther down you go. This chart is usually known as the Snellen chart.

Yet how much do you really know about this eye chart? Are all eye charts the same? How are these eye charts used? And when were they invented?

Here’s everything you need to know about eye charts and more!

What is an Eye Chart?

An eye chart is one of the tools your eye doctor uses to assess your eyesight. Based on how well you can see various letters on the chart, your optometrist will determine whether you have myopia (nearsightedness), hyperopia (farsightedness), presbyopia (age-related farsightedness) or astigmatism, and will measure the prescription that will give you the clearest, most comfortable vision.

Are All Eye Charts The Same?

There are a number of variations to the standard Snellen eye chart. The one an eye doctor uses depends on the personal needs and abilities of the patient. For example, eye doctors will use charts with pictures or patterns for younger children who may not have learned to read or identify letters and numbers.

There are also certain charts that specifically measure distance vision, while others are better for measuring near vision.

History of the Snellen Eye Chart

The Snellen eye chart was developed by Dutch eye doctor Hermann Snellen in the 1860s. Before this standardized eye chart was developed, each eye doctor had their own chart that they preferred to use.

Having so many different eye charts made it impossible to standardize the vision correction available to patients. Eyeglass makers didn’t receive the defined measurements they needed to accurately design, manufacture and measure the optical prescriptions their patients needed.

For the first time, the Snellen eye chart allowed a person to provide a standardized prescription from any eye care provider they chose to any eyeglass maker, and get the same optical lenses to accurately correct their vision.

How The Snellen Chart Is Used in Eye Exams

The standard Snellen chart displays 11 rows of capital letters, with the first row consisting of a single large letter. The farther down the chart you go, the smaller the letters become.

Your New York City & Scarsdale eye doctor will ask you to look through a phoropter – an instrument used to test individual lenses on each eye during an eye exam – and look at the Snellen chart placed 20 feet away. Your eye doctor will prescribe the lenses that provide you with the clearest and most comfortable vision.

In many offices, where 20 feet of space may not be available, you’ll be asked to view the chart through a mirror. This provides the same visual experience as if you were standing 20 feet away.

If you have 20/20 vision, it means you can see what an average person can see on an eye chart from a distance of 20 feet. On the other hand, if you have 20/40 vision, it means you can only see clearly from 20 feet away what a person with perfect vision can see clearly from 40 feet away.

If you have 20/200 vision, the legal definition of blindness, this means what a person with perfect vision can see from 200 feet away, you can see from 20 feet away.

Does 20/20 Visual Acuity Mean Perfect Vision?

No. While eye chart tests identify refractive errors, they can’t detect signs of visual skill deficiencies or diseases such as glaucoma, cataracts or macular degeneration.   • Eating a balanced diet rich in fiber, fruits and vegetables

  • Not smoking, and avoiding 2nd-hand smoke
  • Wearing sunglasses to protect your eyes from ultraviolet (UV) rays
  • Maintaining normal BMI with regular exercise
  • Regular visits to your eye doctor as recommended

What health conditions can an eye exam detect?

A comprehensive eye exam can often detect certain underlying diseases that can threaten your sight and eye health, such as diabetes, high blood pressure, high cholesterol, tumors, autoimmune conditions and thyroid disorders. This is why having your eyes checked regularly is key. The earlier the diagnosis and treatment, the better the outcome and the higher your quality of life.

Bloodshot Eyes – Should You Be Concerned?

You wake up in the morning ready to start your day, only to discover that your eyes are bloodshot. That might not be surprising if you stayed up late to finish a project, had too many drinks at a party or spent time in a smoke-filled room.

But bloodshot eyes can also signal an underlying eye problem. If your eyes appear red or bloodshot, make an appointment with an eye doctor for a comprehensive eye exam to determine the cause and to receive effective treatment.

Why Do I Have Bloodshot Eyes?

When blood rushes to the front of the eye, the tiny red blood vessels on the white of the eye dilate and become visible. This makes the eyes appear red and irritated.

So why do these blood vessels dilate, causing your eyes to look bloodshot?

Bloodshot eyes tend to be caused by:

  • Dry eyes
  • Irritants such as smoke, pollen and perfume
  • Lack of sleep
  • Excessive alcohol consumption
  • Spending too much time in front of the computer

Bloodshot eyes due to lifestyle and environmental irritants may disappear on their own, or you can try to relieve them with over-the-counter eye drops or liquid tears. Lifestyle changes, such as getting more sleep, cutting down on alcohol intake and limiting screen time can often be helpful. If allergies are the culprit, oral antihistamines and antihistamine eye drops may relieve symptoms.

At other times, underlying problems requiring prompt medical attention can cause your eye’s blood vessels to dilate. The following are some of these medical conditions:

Conjunctivitis

You’ve probably heard of “pink eye.” It’s another name for infectious conjunctivitis – an infection of the conjunctiva, the thin membrane covering the eyelid and the front surface of the eye.

There are two types of infectious conjunctivitis – bacterial and viral.

If your child has conjunctivitis, they’re not alone. About 12% of kids get bacterial conjunctivitis every year. This highly contagious condition affects children and adults. In addition to reddish eyes, the following symptoms are associated with conjunctivitis:

  • Bacterial conjunctivitis – irritated eyes, swollen eyelids, eye discharge, crusty eyelids and excessive tearing
  • Viral conjunctivitis – cold or flu-like symptoms, runny nose, fever, itchy eyes, excessive tearing

If you or your child are experiencing these symptoms, it’s important to schedule a prompt appointment with an eye doctor, who can diagnose whether the conjunctivitis is viral, bacterial or due to allergies.

Depending on the diagnosis, your eye doctor will prescribe antibiotic eye drops or creams to treat bacterial conjunctivitis. The viral form may run its course after a few days, but cold compresses and non-prescription eye drops may provide relief.

Dry Eye Syndrome

If your eyes are chronically bloodshot you may have dry eye syndrome (DES). Signs of DES include:

  • Dry, irritated eyes
  • Burning or stinging eyes
  • Discharge from the eyes
  • Light sensitivity
  • A feeling you have something stuck in your eyes
  • Blurred vision
  • Watery eyes

Dry eye syndrome is most commonly caused by a blockage of the tiny meibomian glands in the eyelids. These glands secrete oil that keeps eye moisture from evaporating too quickly. Without the oil, tears dry fast, leaving your eyes feeling dry, itchy and with a bloodshot appearance.

Too much screen time, aging, certain medications such as antihistamines, and medical conditions such as Sjogren’s syndrome can cause dry eye syndrome.

In addition to any medications or in-office treatments your eye doctor recommends, make sure to get plenty of hydration, take frequent breaks from digital screens and use a humidifier in your home.

Uveitis

In addition to bloodshot eyes, if you also experience blurred vision, see floaters or your eyes feel painful, you may have an eye inflammation called uveitis. The causes of uveitis include:

  • Autoimmune or inflammatory condition
  • Infection
  • Medication side effects
  • Cancer (in rare cases)

Unfortunately, uveitis symptoms can often be mistaken for something less serious. That’s the reason it’s important to get an eye exam if your eyes are bloodshot. Left untreated, uveitis can lead to serious conditions such as retinal scarring, cataracts and vision loss.

Depending on the cause and severity, your eye doctor may treat uveitis with prescription eye drops, steroid pills, injections or eye implants.

Eye Injury

It’s vital that all eye injuries receive immediate eye care from an eye doctor.

Even a minor eye injury can cause a big red blotch to form on the white part of the eye (sclera). The cause is a broken blood vessel or a subconjunctival hemorrhage.

Although the appearance of this blood looks severe, and can make the entire white part of the eye appear bright red, a subconjunctival hemorrhage is usually painless and doesn’t cause vision loss. Any time you notice excessive blood on the eye following an eye injury, schedule an appointment with an eye doctor to assess the health of your eye.

Glaucoma

In rare cases, bloodshot eyes may signal the presence of glaucoma – a leading cause of vision loss and blindness.

While some types of glaucoma don’t show symptoms in the early phases, bloodshot eyes can indicate the type of glaucoma that requires immediate medical care. This disease causes damage to the optic nerve due to excessive pressure within the eye. When this pressure suddenly rises, the eye’s blood vessels become dilated and visible, making the eye appear red.

If you have bloodshot eyes and/or have the following risk factors for glaucoma, immediately schedule an appointment with your eye doctor.

  • Family history of glaucoma
  • Aged 60+
  • African American, Asian or Hispanic
  • Diabetes
  • High blood pressure

Can I get bloodshot eyes after LASIK surgery?

LASIK surgery is highly effective minimally invasive laser eye surgery that can correct refractive errors, but like all surgical procedures, it can have side effects. Your eyes may be bloodshot or you could see halos from a few days to three weeks after surgery. Additionally, you may experience other dry eye symptoms. Eye drops and liquid tears can alleviate these symptoms, but if you have any concerns about your eyes following LASIK surgery contact your eye surgeon.

What Should I Expect from a Glaucoma Exam?

If you have a family history and/or other risk factors for glaucoma, and if your eyes look bloodshot, consider scheduling a glaucoma exam. Your eye doctor may perform the following tests:

  • Tonometry – eye pressure test
  • Gonioscopy – to see how fluid is draining out of your eye
  • Vision field test – to examine the functioning of the optic nerve
  • Dilated pupil exam – to detect any damage to the optic nerve
  • Retinal photo or OCT – digital examination of the retina and optic nerve health

What’s Your Optometrist Role in Cataract Surgery?

If you’re over the age of 60, there’s a good chance you’ll develop cataracts sometime in the next 20 or so years. While the only effective long-term treatment for cataracts is surgery, it can take years or even decades for a cataract to reach the point where it needs to be surgically removed.

In the meantime, your optometrist can monitor its progression, manage your symptoms and ensure you have the best vision possible. Once your cataract makes it difficult for you to function day-to-day, your eye doctor will refer you to an ophthalmologist who will perform eye surgery to replace your eye’s natural lens with a clear artificial lens.

Following your surgery, your optometrist will co-manage your post-op recovery in coordination with your eye surgeon.

Your Optometrist Will Discuss Cataract Treatment Options

The Importance of Regular Eye Exams

Regular eye exams are important for everyone, and particularly if you’ve been diagnosed with cataracts. What’s the best treatment for cataracts?

How Pregnancy Can Affect Your Eyesight

Pregnancy can impact almost every part of a woman’s body and health — including her eyes. In fact, an estimated 14% of pregnant women report experiencing visual changes during pregnancy that usually resolve on their own within a couple of months after giving birth.

Knowing the different visual symptoms that can present when you’re expecting can help alert you to potential underlying health concerns that your physician may need to address.

Normal Visual Changes During Pregnancy

Blurred Vision

Blurred vision is the most common visual symptom that pregnant women may experience. Hormonal fluctuations are usually to blame for the temporary decrease in visual acuity, and your eyesight will likely return to normal soon after giving birth.

The influx of pregnancy hormones causes fluid retention in some areas of the body and can cause the cornea to thicken slightly. As a result, the light entering the eye isn’t focused accurately and vision may be blurred.

Less commonly, blurred vision can signal gestational diabetes, a pregnancy complication affecting 6-9% of pregnant women. The rise in blood sugar level impacts the focusing lens of the eye, leading to blurry vision. If you are diagnosed with diabetes, including gestational diabetes, it’s a good idea to book an eye exam to monitor for retinal changes.

Blurred vision is also a common side effect of dry eye syndrome, a condition characterized by tears that don’t adequately lubricate the eyes, which can be brought on or exacerbated by pregnancy.

Eye Dryness

Pregnancy hormones can cause a reduction in the amount of tears your eyes produce or affect the quality of the tears. These changes can affect a woman throughout her entire pregnancy, but studies show that eye dryness is particularly common in the last trimester. For this reason, some women find it difficult to wear contact lenses in their third trimester and temporarily switch to glasses.

Eye Puffiness

Yet another body part that swells during pregnancy: the eyelids and tissues around the eyes.

Pregnancy-related water retention may cause your eyelids to appear puffier than during your pre-pregnancy days. You may also notice darker areas under the eyes. If your puffy eyes bother you, try limiting your salt and caffeine intake, as they can worsen the problem.

Visual Changes That May Indicate a Problem

The following visual changes warrant a prompt call to your eye doctor or obstetrician to rule out any underlying complications.

Flashes or floaters

Seeing stars during pregnancy can signal high blood pressure, which is associated with preeclampsia — a serious medical condition that requires close monitoring by your physician and possible treatment.

Why are regular eye exams important?

What Is the Long-Term Impact of Virtual Learning on Children’s Eyes?

Kids, like adults, are spending more time online. At some point during the COVID-19 pandemic, many children attended school via Zoom and completed assignments online. The trend toward more screen time — whether playing games or being in touch with friends — is likely to continue even after everyone returns to the classroom.

We already know that prolonged screen time can cause digital eye strain as well as dry eye symptoms, among other problems in children and adults. There is some indication that extended exposure to blue light may impact the development of retinal cells. However, studies on actual subjects still need to be done to establish a clear connection.

Dry Eyes

Spending a long time in front of screens can impact how quickly our tears evaporate, because we blink around 66% less when using a computer compared to other daily activities. When tears evaporate too quickly and aren’t replenished with blinking our eyes start to feel dry and gritty. So remember to blink every few seconds to prevent your eyes from drying out!

Blue Light Exposure

Screens, such as those that appear on computers, phones and tablets emit blue light. Recent studies have shown that overexposure to blue light can damage the retinal cells at the back of your eyes. This may increase the risk of vision issues such as age-related macular degeneration which eventually leads to permanent loss of vision.

Excess blue light has also been shown to disrupt the circadian rhythms that regulate our sleep patterns, as it tricks your internal clock into thinking that it is the middle of the day. This may lead to difficulty in falling asleep, insomnia, and daytime fatigue.

Digital Eye Strain

Nearly 60% of people who routinely use computers or digital devices experience symptoms of digital eye strain — also called computer vision syndrome. Symptoms of eye strain include eye fatigue and discomfort, dry eye, headaches, blurred vision, neck and shoulder pain, eye twitching, and red eyes.

Taking frequent breaks from your screen can help reduce eye strain and neck, back and shoulder pain during your workday.

It is recommended to take at least one 10-minute break every hour. During these breaks, stand up, move about and stretch your arms, legs, back, neck and shoulders to relieve tension and muscle aches.

Also, look away from your computer at least every 20 minutes and gaze at a distant object at least 20 feet away for at least 20 seconds. This relaxes the focusing lens inside the eye to prevent fatigue.

What’s the 20-20-20 rule?

If you find yourself gazing at screens all day, whether your computer, smartphone, iPad or television, you’re at risk of experiencing eye strain. So make sure you schedule frequent breaks from your screen and follow the 20-20-20 rule; every 20 minutes, look at something 20 feet away for 20 seconds. Keratoconus and Contact Lenses

In its early stages, people with keratoconus can usually wear glasses or standard soft contact lenses to correct resulting astigmatism and improve clarity. As the condition progresses, however, your eye doctor will prescribe the most suitable contact lens to accommodate a cone-shaped cornea to provide you with the clearest vision possible.

There are a variety of contact lens options for keratoconus, all of which depend on the severity of the condition.

1. Soft Toric Lenses

Astigmatism can be corrected with soft toric lenses, a comfortable and effective solution during the early stages of KC. However, this may not be a good treatment option as the condition worsens.

2. Custom Soft Contact Lenses

Custom soft contacts are an improvement over soft toric lenses since they are designed to match the exact contour of your cornea. Soft contact lenses can be custom-made for KC patients and may eliminate the need for glasses by fully correcting astigmatism.

3. Gas Permeable Contact Lenses

Gas permeable contact lenses (hard lenses) allow more oxygen into the eye than standard soft lenses. A gas permeable lens has a somewhat different shape than soft contact lens alternatives, as it rests on the cornea, and because the lens is hard, it creates a new optical surface. These lenses are suitable for people with moderate KC.

4. Hybrid Contact Lenses

These lenses are a cross between hard and soft lenses. The hard center provides a flat surface, which helps alleviate the issues associated with a misshapen KC cornea. The lens is rendered more comfortable by the soft outer ring. Hybrids combine the convenience and comfort of soft lenses with the crisp, clear vision of rigid gas permeable contacts.

5. Scleral Lenses

Scleral lenses are larger than standard hybrid, gas permeable or custom soft contact lenses. They vault over the cornea and rest on the sclera, the white of the eye. The advantage of scleral lenses is that they do not sit on the cornea, which removes any rubbing and irritation on the corneal bulge. This reduces the risk of corneal injury from the contact lens.

Furthermore, as the scleral lens vaults above the cornea, the reservoir of pure saline solution between the underside of the lens and the front of the cornea keeps the eye in a liquid environment at all times. This enables the eye to receive an abundance of oxygen.

While both rigid gas permeable (GP) and scleral lenses deliver enough oxygen to the eyes, scleral lenses provide more comfort and better stability than regular GP lenses. For this reason, scleral contact lenses are a very successful option for people with keratoconus and irregularly shaped corneas.

Can Vision Therapy Help Those With Autism 640×350Visual problems in autistic children commonly go undetected and untreated. Often mistaken for symptoms of autism, visual problems can make it much more difficult for individuals with autism to process what they are seeing.

In a 2019 review of eye clinic records, the Journal of the American Association for Pediatric Ophthalmology and Strabismus found that many autistic children have undetected vision problems.

“Among 2,555 children at a university autism clinic, about 11% had significant vision disorders, including strabismus (eye misalignment) and amblyopia, in which poor vision in one or both eyes results from abnormal early visual development,” the researchers said.

Though many of the following autistic behaviors may appear to be unrelated to vision impairment, in reality, a high number of them are due to poor vision or visual skills.

Below, we answer some of the most frequently asked questions about astigmatism and explain why scleral contact lenses are often prescribed to astigmatic patients.

1. What is Astigmatism?

Astigmatism is a common refractive error caused by a cornea that isn’t perfectly spherical. The cornea is the outer front covering of the eye and is partially responsible for refracting light onto the retina. When the cornea is misshapen, it refracts light incorrectly, creating two focus points of light entering the eye. Since the light is no longer focused on the retina, it results in blurred vision at all distances.

2. What are the Symptoms of Astigmatism?

The main symptom of astigmatism is blurred vision, but it can also cause symptoms like:

  • Objects appearing wavy or distorted
  • Squinting
  • Headaches
  • Poor night vision
  • Frequent eye strain

3. How Common is Astigmatism?

Astigmatism affects approximately 1 in 3 individuals around the world. Most people with myopia (nearsightedness) or hyperopia (farsightedness) also have some level of astigmatism.

4. What’s the Difference Between Astigmatism, Nearsightedness and Farsightedness?

Although all 3 of these refractive errors negatively affect visual clarity, they are caused by different mechanisms.

Astigmatism is a result of a non-spherical cornea, which causes two focal points and blurry vision. Myopia occurs when the corneal focusing power is too high and the light focuses in front of, instead of directly, on the retina. Hyperopia occurs when the corneal power is too weak, so the light rays focus behind the retina, not on it. Both myopia and hyperopia can occur with a spherical cornea.

5. How is Astigmatism Corrected?

In cases of mild to moderate astigmatism, the blurred vision can be easily corrected with prescription glasses or contact lenses. But for patients with high levels of astigmatism, standard contact lenses may not be an option. Toric contact lenses are a popular choice for patients with mild or moderate astigmatism due to their unique focusing features and oblong shape. Scleral contact lenses are suitable for moderate to severe astigmatism.

Refractive surgery is also an option, but comes with the risk of surgical complications.

6. Why Can’t Individuals With High Astigmatism Wear Standard Contact Lenses?

A highly astigmatic cornea has an irregularly shaped surface that isn’t compatible with standard soft contact lenses. Standard soft lenses are limited in the amount of astigmatism they can correct, as these lenses move around on the cornea due to the cornea’s irregular shape. This, in turn, reduces visual clarity and comfort.

Regular hard lenses can often correct astigmatism better than soft lenses, but they, too, have limitations: these lenses are smaller and may also move around too much.

7. Why are Scleral Lenses Ideal For Astigmatism?

Scleral contact lenses are customized to each patient. They have a larger diameter than standard lenses, and thus cover the entire front surface of the eye. These specialized rigid lenses gently rest on the white part of the eye (sclera) and don’t place any pressure on the sensitive cornea, making them suitable for even highly astigmatic eyes.

Furthermore, scleral contact lenses contain a nourishing reservoir of fluid that sits between the eye and the inside of the lens, providing the cornea with oxygen and hydration all day long. In fact, patients typically report that sclerals provide sharper vision than other types of contact lenses.

What Happens If You Don’t Treat Dry Eye Syndrome?

girl sitting in the pool 640×350Dry eye syndrome (DES) is a condition where your eyes either produce low-quality tears or don’t produce enough tears to keep your eyes hydrated. Sometimes the meibomian glands inside your eyelids, which produce the oily layer of your tears, don’t function properly or are blocked, causing your tears to dry out. Environmental factors, certain medical conditions and several medications can also cause DES.

Tears are essential for maintaining eye health and comfort. They moisten your eyes and remove debris. In severe cases, untreated dry eye syndrome can actually damage your cornea and cause vision loss.

The amount of dryness varies in severity from person to person. If you have a minor case of dry eye, you may be able to manage it with over-the-counter eye drops. However, if the problem persists or appears to be getting worse, it’s time to visit your eye doctor, who will assess your eyes, find the underlying problem and offer treatment for lasting relief.

Below is a list of complications that may occur if chronic dry eye syndrome isn’t treated:

Conjunctivitis

Conjunctivitis refers to infected or inflamed conjunctiva — the clear layer of cells that covers the white part of your eyeball and the inner surface of your eyelids. Symptoms include grittiness, redness and sensitivity to light.

Keratitis

Keratitis refers to an inflammation of the cornea. It can be caused by different types of infections, abnormalities of the eyelids, injury and dry eye. If the deeper layers of the cornea are involved, scarring or a corneal ulcer may result, particularly if left untreated.

Corneal Ulcer

A corneal ulcer is an open sore that develops on the cornea—the clear, protective outer layer of your eyes.

While corneal ulcers typically develop following an injury, they can also be caused by severe dry eye.

On a daily basis, debris, like dirt and sand particles, enter your eyes and scratch the surface of the cornea. When your tear glands don’t produce enough tears to wash away the particles, bacteria can infect the scratch and cause an ulcer.

Luckily, corneal ulcers are easily treated with antibiotic eye drops. Left untreated, however, these ulcers can spread and scar the eyeball, causing partial or even complete blindness.

Inability to wear contact lenses

Unless your eyes produce enough good-quality tears, your contact lenses can become overly dry, leading to a gritty sensation, irritation and redness. Without sufficient moisture, your contacts may stick to your eyeball, making it difficult to remove them.

Though chronic dry eye syndrome may prevent you from wearing standard contact lenses, certain specialized contact lenses can improve ocular hydration and comfort.

Difficulty keeping your eyes open

Depending on the severity of dry eye, it may be difficult to keep your eyes open. This may occur if dry eye syndrome causes extreme light sensitivity or a chronic sensation that something is stuck in your eye.

While artificial tears may provide enough moisture to partially open your eyes, you may still feel the urge to squint, especially when exposed to a computer screen or sunlight.

Difficulty reading or driving

While blurred vision often signals that you need a stronger prescription, it’s also a common symptom of chronic dry eye syndrome.

Left untreated, the blurriness may worsen and even lead to double vision. Naturally, this makes driving and reading a real struggle.

Headaches

While there’s room for more research, studies have shown that there may be a connection between headaches and dry eye. A population-based case study of more than 72,000 patients published by JAMA Ophthalmology (2019) found that people who suffer from migraine headaches are more likely to have dry eyes compared to the general population.

It’s not clear why. According to the paper, being female and of advanced age play an important role in determining the strength of this association.

Depression

A 2015 study, published in the journal Cornea evaluated the connection between dry eye disease and depressive symptoms in more than 6,000 women. Researchers found that women diagnosed with dry eye had a higher likelihood of developing depressive moods, anxiety, and psychological stress.

While the connection isn’t fully understood, researchers noted that some medications for treating depression have a drying effect on the eyes, and that dry eye syndrome may limit a person’s participation in activities, to the point where they may become anxious, withdrawn and even depressed.
